Cuba Travel Registration Checklist

Please complete the items on this checklist only if you received instructions from Ocean Doctor to do so.

IMPORTANT: Please complete steps 1-4. Once all documents and payments are received, we will send you an email to indicate that your reservation is confirmed. Only after you have received our email confirming your reservation should you proceed with steps 5 and (optionally) 6.

  • Travel Registration Form

    The registration form provides us with the name, contact information, passport information, rooming preferences, allergies, etc. for each traveler.

  • Booking Agreement

    The travel agreement includes the terms of conditions of travel. You may e-sign this agreement online. A separate signed agreement is required from every traveler.

  • Liability Release

    The liability release delineates Ocean Doctor’s responsibilities and limits of liability. You may e-sign this agreement online. A separate signed release is required from every traveler.

  • Payment

    Make your deposit, balance or other payments to Ocean Doctor here. Payment instructions are provided in your booking agreement and on the payment page. Your reservation cannot be confirmed without your deposit (your first payment). (Note: Your air travel and Cuban visa will be arranged by our Travel Service Provider, Marazul. Payments for your airfare and visa will be made directly to Marazul — see next step.)

  • Air Travel & Visa Forms

    IMPORTANT: COMPLETE THIS STEP (AND SUBSEQUENT STEPS) ONLY AFTER RECEIVING THE CONFIRMATION OF YOUR RESERVATION FROM OCEAN DOCTOR. Download, complete and submit your forms to our Travel Service Provider, Marazul who will book your  roundtrip air travel directly from your home city to Havana and provide you with your Cuban visa.   • Travel Insurance (Optional)

    Included in the price of your airline ticket is Cuban health insurance coverage for the duration of your stay. Check with your own health insurance company regarding coverage while you are in Cuba.  We recommend travel insurance for your trip, such as trip interruption, baggage delay, flight delay, etc. For our own travel we often consult SquareMouth, an online comparison engine that allows you to compare more than 100 different travel insurance policies.
